	All new food service facilities must submit a plan review and an application for a license to operate to the Health Department with three sets of building plans and the necessary fees. Effective July 1, 2007 the fee for the food service plan review application is $80.00, the Class II License fee is $135.00 and the Class IV License fee is $268.00. Please submit separate checks for the Plan Review and the License Application fees. Checks are payable to the Town of Greenwich.
